Summary

Care Coordinator

Join our dynamic team as a Care Coordinator - a key role in the successful implementation of the Certified Community Behavioral Health Clinic (CCBHC). A Certified Community Behavioral Health Clinic (CCBHC) is designed to improve timely access to a full range of mental health and substance use treatment services offered to patients and clients regardless of insurance through continuous quality improvement and strategic community partnerships.

Responsibilities:

Required Minimum Knowledge, Skills, and Abilities (KSAs) - "What you will need":

  • Education: Bachelor in social work or related field preferred
  • License/Certifications: None required
  • Experience: Minimum of two years' experience working with behavioral health consumers and families. Experience working in team based settings and collaborating with multiple providers preferred.
