Job Description

Summary

  • Amazing opportunity to work in a dynamic community-health setting with the embedded case manager in a health-coach capacity.

  • The Health Coach is a trusted member of the care management team who provides individualized, compassionate wellness coaching and support to patients identified for care management. In collaboration with the licensed care manager or designee, the Health Coach will partner with patients to ensure improved access to, and coordination of, clinical and community services across the continuum of care. This position will involve carrying a defined caseload of patients who are being supported to achieve specific concrete goals aimed toward improving the patient's overall health and wellness.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: High School Diploma preferred. Bachelor's Degree preferred.
    • License/Certifications: Valid Maine Driver's License. Health Coach certification preferred.
    • Experience: 2 years' working in a social service or health care setting or equivalent educational setting preferred.
    • Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
    • Bilingual preferred.
    • Sensitivity to cultural and ethnic diversity preferred.
    • Local community resident with good knowledge of the resources available in the community preferred.
    • Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
    • Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
    • Familiarity with medical terminology preferred. Ability to project a pleasant and professional image even under stressful situations. Ability to maintain confidentiality.
    • Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
    • Ability to travel frequently to PCP practices and other sites where patients receive care. Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.
