Job Description

Summary

  • Position Summary
    • Within MaineHealth's ambulatory pharmacy program, the clinical pharmacist will be a part of our growing Hemostasis and Thrombosis program. In this role, the pharmacist will help provide medication therapy for anticoagulant, antiplatelet, and hemostatic agents. Based initially within an active MaineHealth Cardiology ambulatory office, the clinical pharmacist position will be focused on providing high quality direct patient care through collaborative drug therapy management. Additionally, the clinical pharmacist will collaborate closely with MaineHealth's primary care pharmacists as well as inpatient pharmacists and will support the transition of care for clinic patients.

      Some core functions include:

    • Works collaboratively with all healthcare professionals to provide safe and cost-effective patient care, including participation in collaborative drug therapy management.
    • Communicates directly with patients and their families on all aspects related to medication therapy promoting high levels of knowledge, engagement, and adherence.
    • Ensure appropriate and optimized medication therapy for patients with thrombotic disorders including venous thromboembolism (VTE), atrial fibrillation (AF), mechanical heart valves, and prevention of VTE in high-risk patients.
    • Provides education to other healthcare professionals, including precepting trainees to enhance clinical competency on topics related to anticoagulation therapy.
    • Partners with other healthcare professionals to develop evidence-based clinical pathways.
    • Generates and analyzes process and outcome measures for use in assessing medication related patient outcomes.
    • Demonstrates ongoing professional development including meeting continuing education requirements, keeping abreast of current professional literature and how it relates to clinical, financial and safety outcomes.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: Graduate of an ACCP accredited school/college of pharmacy is required.
    • License/Certifications: Eligible or currently licensed in the State of Maine.
    • Experience: Work experience as a patient care pharmacist involved in patient monitoring and drug therapy review is preferred. Experience in an organized healthcare setting is preferred. Experience in the monitoring and consultation on drug therapy in patients of all age groups.
    • Excellent verbal and written communication skills are required.
    • Analytical skills necessary to identify and resolve problems related to drug therapy.
